Services may be covered in full or in part by your health insurance or employee benefit plan. Prior to your evaluation, my office staff will check benefits for testing and provide you with the information given to us by your insurance company. Please note that all insurance companies reserve the right to cover or deny services based on a number of factors, and while we do our best to provide you with an estimate of coverage and any out of pocket expenses, this is not a guarantee of coverage.
Below is a list of insurance plans I participate with. This is not an exhaustive list, so if you have a different policy, please contact the office staff to see if services are covered under your plan.

Cash, check and most major credit cards (Visa, MC) accepted for payment.
Please contact the office as soon as possible if you are unable to make your appointment. There is a 24 hour answering service so calls for cancellations can be taken at any time. Please be advised that rescheduling evaluations can take several weeks.
